With the tournament only days away, it is time to make sure you have everything you need to have team check-in run quickly & smoothly for you.  Please follow the list below and ensure all is done and ready to bring with you to team check-in.

By Late Sunday, 8/28/11:

• Ensure all of your players have been added to your roster in your 2011-12 Team Account.
   
• Make sure you have input each player’s jersey number in your team account (including your guest players)
   
• For any Guest/Loan Players, you will need to add that player to your tournament roster in your Got Soccer team account. Click here for the Easy 2-Step Process to add Guest Players - that must be done for all guest players - max of 5 (even those from your own club).

Then bring the following with you to the Mandatory Team Check-In on Friday, 9/2/11:

• 2011-12 Official Association Roster (FYSA, US Club, AYSO, etc) – One copy.  This is not the list of players you see in your team account under the Roster tab.  It is the Official Roster produced through your association that includes the association’s logo.
  
  
  • Note:  U16+ teams may have up to 22 players rostered.  For U16+ teams that have more than 18 rostered players, you check in your full roster on Friday night, but then you must declare the 18 that will participate in each game with the administrative tent staff near your field AT LEAST 30 minutes prior to each game.  
    
    
• For each Guest Player, if any, provide a 2011-12 Guest Player / Player Loan Form (obtained from the player’s home club - even for those that are guesting from your own club).

• 2011-12 PLAYER PASSES (including any Guest Players) and COACHES PASSES - Signed, pictured and laminated

• NOTARIZED MEDICAL RELEASE FORMS - for each player for review (we don’t need a copy)

• Completed COUNTY SURVEY FORM - print & complete prior to team check in to speed up your check in time.
   
• Completed LOCAL CONTACT INFORMATION FORM - print & complete prior to team check in to speed up your check in time.
